What is A Superbill?

We are a cash-based practice, which means you pay us directly at the time of service, and then we give you the tools you need to seek reimbursement from your insurance company. One of those tools is called a superbill.

Think of a superbill as a detailed receipt that has all the information your insurance company needs to consider your claim. Below is a step-by-step guide to help you submit it and increase your chances of getting reimbursed.

What Does “Cash-Based Practice” Mean?

At Mountain Movement Chiropractic, being cash-based doesn’t mean we only take cash. We accept all major credit cards, debit cards, and checks. “Cash-based” simply means that we don’t bill insurance on your behalf. Instead:

  • You pay at the time of your visit.

  • We provide transparent pricing upfront.

  • You stay in control. No surprise bills months later.

This model keeps our care simple, affordable, and free from the red tape of insurance contracts. The superbill gives you the option to still involve your insurance company if you choose.

Step 1: Request a Superbill

We don’t automatically provide a superbill at each visit. If you want to submit to your insurance, you’ll need to ask us for one. When requested, we’ll provide you with a document that includes:

  • Your personal and insurance information

  • Our clinic details (name, address, tax ID, NPI)

  • Diagnosis codes (why you were treated)

  • Procedure codes (what we did)

  • The amount you paid

This document is what insurance companies require to process your out-of-network claim.

Step 2: Contact Your Insurance Company

Before you submit, it helps to call your insurance company and ask:

  • Do you cover out-of-network chiropractic care?

  • What percentage do you reimburse?

  • Where do I send my superbill?

  • Do I need a specific claim form to go with it?

Getting clear answers up front prevents wasted effort later.

Step 3: Fill Out a Claim Form

Most insurance companies require a short claim form to be submitted with your superbill. You can usually find this form on their website or request it by mail. Fill it out, attach your superbill, and keep a copy for your records.

Step 4: Submit Your Claim

Send your claim form and superbill to the address or portal your insurance company provides. Many companies allow digital uploads, which can speed things up.

Step 5: Wait for Reimbursement

Processing times vary, but typically you’ll hear back in 2–6 weeks. If your claim is approved, the reimbursement will go directly to you, not to our office.
